Upgrade can also mean upgrade to a higher fare, not to a higher cabin. As an example, if you change a T ticket just a few days before the date being changed to, you will be caught by the advance purchase rule, and will have to upgrade to a W fare (or whatever fare allows you to buy a ticket a few days before flying).
