If someone else encounters a similar problem, I did three things yesterday:
- called NWA and had them change the AF prefix of the FB account number to a KL one
- reconfirmed the reservation online
- entered my contact and immigration details

Simply enter your FQTV details under AF-KL FB and things will work just fine. I have 8 upgraded segements currently and they all cleared at the 6-day window.
If for some reason the status is not reflected properly, simply make sure it pulls data again which can be most easily done by removing the info and then re-entering it.
