Don't worry about a notarized letter, just write and sign one on your own, based on the template mentioned above... The only time I have ever been asked, and one of my daughter's has a different last name than both her mom and I, is when re-enterring Canada.

Don't even worry about getting it witnessed, as that really does not mean a thing!
