Best rewards card for each common category of spending?

I'm sure I'm not the only FTer who has a flowchart to determine which card to use for different situations and different categories of merchants. Much of the time, it's specific categories of merchants that makes using a specific card worthwhile. For instance, many give a considerably higher rebate or points for gas, grocery stores, etc. I have many categories where I suspect that there are cards that would give me better rewards.

So here's where we can all share. If you have or know of a card that gives a higher rebate or reward for merchants in specific categories, post the details here.

I'll reserve the first response and will try to keep it updated regularly with a summary of the information provided. I'll try to link to the appropriate posts or threads so people can get more detail.

A few ground rules:

1) Let's keep it on topic. Debates about which card is better or how worthless some card is can go in other threads.
2) Only post cards that pay a higher rebate or rewards for specific categories. If it pays the same amount for everything, it doesn't belong here. If a card pays higher than most, I'll include them in an "Everything Else" category.
3) If it pays a higher rebate or reward for a specific brand (like the Hilton Amex for Hilton stays) I'll put that in a separate place so we don't clutter the category listings.
4) If it's a temporary thing (like Discover's quarterly 5% rebate categories), I'm not sure if that's worth keeping track of or not.
5) Try to note any significant restrictions. For instance, if there's a cap on the higher earnings.
6) If there are standard restrictions for the category (like "grocery stores" typically excludes warehouse stores), make sure to point out if any of the standard restrictions don't apply.
7) If the card has no annual fee, let me know and I'll highlight it in green, since these may be more desirable to many people.
8) If you see something wrong or outdated, leave a reply so I can update it.
