The pending queue is not the waitlist. Pending applies only to e-500 upgrades that have been requested but are still outside the clearance window. Once the clearance window opens up, the pending requests are either cleared or moved onto the upgrade waitlist.

Only for a 1K. If you're not a 1K, your mileage upgrade request goes on the PB waitlist.

No. The only difference status makes prior to the DM is for 1K mileage upgrades. All SWU and CR-1 upgrades go on PA. 1K mileage upgrades go on PA (based on the status of the sponsor, not the traveler). All other mileage upgrade go on PB. When a 1K requests any confirmable upgrade, the 1K will go to the end of the PA waitlist, regardless of who else is on it. So if a 1P or even a GM is on PA, the 1K will not jump in front.
