Believe it or not, I still have a couple of the Above & Beyond cards, which I think date back to the 90's. AFAIR I've handed out two and the recipient always seemed to appreciate it.

I've used the on-line feedback form for a mixed praise/complaint message (praise for a FA, complaint for a GA) and got no acknowledgement whatsoever.
