I did see a page similar to the attachment -- but this one specifically mentioned the $100 would apply to Gold/Plats as well. There were a series of questions about how it would impact my choice of hotels and impression of the MR program. There was also a set of follow-up questions about how one would feel if there was a $100 charge for the lounge but a 30% discount for Gold/Plats.
Unfortunately, I didn't take a screenshot of the survey while I was taking it.
