I cannot answer number 2, but I guess numbers 1 and 3 are somewhat related.
A fuel buster is a strategy to remove the fuel surcharge (also known as YQ). You will not find an obvious answer because there have been previous instances where FT lurkers have taken advantage of a deal but do not know about the etiquette of a fare mistake and/or fuel buster; these folks would call in while the deal is still live and would ask for stupid things like seat assignment and even "will UA charge me for the missing YQ"? The etiquette of course is not to call in while the deal is live - the fewer pairs of eyes on the fare, the higher the chances will be for others to take advantage... So now, most people on FT who have lived through the whole sage would know what a fuel buster is but it appears you missed it.
