IME, no. If you get re-routed while in transit for any reason, you should be able to get the original flight credit. However, sometimes it's best to take the re-route credit if it takes you farther out of your way.

It shouldn't be. Once you mis-connect, you should be able to re-route to get to your destination as close to on time as possible. Often, this requires going stand by though.

I must have missed where this was international... As I read it, the OP's route was DCA-ORD-LAX-HNL which is all domestic.
