i believe that, if you already have another icc card (plat, etc), you can call up amex and ask them what the requirements are. the fee is 2800 usd or 2800 eur, depending on the base currency you choose.

this site says that icc centurion is by invite only. each icc card (green, gold, or plat) has its own requirements and i suspect you need to be a member of plat before you can be considered for centurion.

unless...

you have your private bank (if it is an icc partner) submit your centurion application on your behalf. these partners have a lot of 'pull' with amex, most usually ensuring that an application is approved. this route usually means no consideration for minimum spend or, in fact, anything...
