In fact, there are Americans who travel outside the United States to be treated for an array of health issues, and it can cost less for the treatment, including all travel expenses, than for similar treatment in the United States.

This is a good point. I was thinking in terms of civilised countries (which will hopefully someday soon include the US) who will cover the costs of necessary medical care for their citizens.
