The Ritz-Carlton is definitely a luxury hotel; I don't really think there's any question about this.

The reviews about the RC Tokyo are no more 'mixed' than the MO NYC to my eye.

Personally, I think the Grand Hyatt just straddles the boundary between luxury and merely very upscale as well; although it's ruled out by the OP's criteria.

Based on those criteria, I would say that the RC Tokyo is probably an excellent choice.
