No, they dont subtract the $150 from the voucher. The $150 is ticket change fee that would be charged when he REBOOK his trip and use the voucher to pay.
AA actually is better than some other airlines, in that the ticket change fee is deducted from the voucher, and if you need to do counter ticketing, or phone ticketing, AA would not charge you the additional ticketing fee.
In the case of CO, you would need to pay the $150 AND any ticketing fee separately via your credit card when you re-ticket.
