I use exactly those shown in the top picture of the Ziploc Wikipedia article:

Quite sturdy, and each lasts a few months. Usually a few tiny wholes start appearing after a few months because the toothpaste cuts them, but they are too small for anything to fall out. I always carry one or two spares.
