I just got the last piece of my signup bonus from the NW Visa -- total bonus was 25k miles.
I recently saw a 15k signup bonus for the NW Visa with the fee waived the first year.
The American Airlines Mastercard gives a 10k signup bonus
and is free for the first 6 months.
Delta Amex often offers pretty lucrative signup bonuses.

I believe it's double miles
on purchases from telephone and mail order merchants only.
