My wife was once half-paying attention as the UA gate agents announced her flight but apparently only gave the final destination rather than her intermediate stop. So she missed it. And more than once I've been delayed when UA or other airlines had to offload luggage for passengers who checked in but didn't board. I guess these things do happen unintentionally from time to time..
