CO never had or has lie-flat seats (unless you consider their angled seats to be flat), other airlines have flat seats in J, but not Continental.

Delta does not fly PHX to CDG directly, you would have to connect in ATL or JFK (not sure about CVG), and even then it wouldn't be 757-200 on the TATL segment fro NA to CDG.

Not sure what you are asking.
