You may get slightly thrown by questions about visiting forbidden regions or requiring referees for your visit. You just put NA in all of these boxes.

You need two blank pages in your passport, these don't need to be facing.

Also worth mentioning that the *appointment* you get means turning up at the same time as about 100 or so other people. The office is a bit like going to a UK social security office. Take a good book and the wait will whizz by.
