I did some searching for this promo when you first posted, and did find this web page, but since there was no mention of 750 bonus miles, didn't think it was what you were looking for. This promo is also in the Tangerine flyer, but again, no mention of 750 bonus miles.

Methinks you will have problems getting it if you don't have a print out of the offer!
