Germany is much more restrictive than the U.S. If fueling is done with passengers on board in Germany, the fire department will respond to the airplane and standby (I've done this twice, once on a ORD-AMS flight that diverted to DUS, and once on a MUC-IAD flight where we needed extra fuel due to an inop HF radio and a new routing).

In the U.S., a fire engine is never needed. As long as the jetway is attached, fueling can proceed as normal. With the doors closed, the slides must be armed and the flight attendants must be at the doors. Otherwise, it's no problem.

We would have no problem landing in Cuba with a fuel emergency, btw. Heck, we fly 767 charters to Cuba all the time...
