I usually receive a survey by email after I emailed to Ambassador Services for a complaint or something else. Here is what the email looks like with subject "Thank you for your Priority Club Inquiry - How did we do?":

Dear GordonGordon,

Thank you for your recent inquiry regarding your Priority Club Rewards account. We would appreciate your honest feedback about that specific experience.

In an effort to continually improve customer satisfaction, Priority Club Rewards has partnered with an independent market research company, Mindshare Technologies, to conduct a survey about the Priority Club Rewards customer service experience.
