MC World Elite isn't itself a card and doesn't have a rewards program, it's just a tier of
MC.

Citibank Premier Pass Elite (and, I've heard, the regular Premier Pass) is World Elite, although that's a recent change so some of the reviews and application materials may not be updated yet. It offers 1 (kinda, PriorityPass basic) and 2 (kinda, via Virtuoso). Its rewards program isn't quite what you describe in 3 but people have made good use of it. However, if you're concerned about 4, I would completely ignore rewards programs; they're for people who pay in full every month and don't care about 4.
