Same with Nurse Practitioners who may be called "Doctor", depending in their level of expertise. Huge debate right now in the US as physicians/surgeons absolutely hate it (they have some valid reasons to do so).

No. Not in Germany.
Dr. is an academic degree not a profession. If you finish your studies you are Arzt="Medical". If someone adresses you as Dr. you have to correct it. Otherwise it is a misuse of a title. You have to write a theses and then you get your Dr. med.
BWT you start as stud.med. Before the exam you are cand.med. and then nothing until you have passed your Doctorial thesis.
BTW I have 2 Diplomas and Dr.phil. wich makes adressing me in Austria a bit crazy.
As Flying Lawyer said the Dr. becomes part of my name (as with Dr. med. Mrs. Tyrolean).
A Dr. does not indicate a medical person. There are many Medicals without and lots of Drs. have not studied medicine.
LH knows that Mrs. Dr. med. Tyrolean is a medical doctor form the aditional passenger information.
It depends if I am adressed with my title if it is correctly put into my booking. If the booking is made on
www.aua.com and the flight is on LH it is sometimes mixed up a bit.
Funny is when I travel with Dr. FahtherOfTyrolean, which I do very often (as we work together).
Usual Conversation: Oh, two Dr. Tyrolean? How come?
Me: Oh we give our children the surname Doctor. Makes it easier in life
