My experience.

Flew TG from KIX to HKT I had to pay fuel surcharge at KIX on reward tix.

Flew NH from KIX to CTS (domestic), no fuel surcharge on reward tix. I think NH only charges for international rewards.

Not surprisingly, each carrier appears to have their own rules.
