you can handle this through a macro. Start recording a macro, set the header and print page one. Stop recording the macro. Go to the macro editor (apt f11). You will see the code that prints one page after changing the headed. Copy and change the code to the information you need for page 2, etc. Then to get fancier, you can even pull the data you need from Excel or Access as needed.

That is supposed to be alt f11. It is near impossible to edit a post on an iPhone.
