You mean the three digit number on the signature panel. (Visa call it CVV2 - Card Verification Value 2, MasterCard CVC - Card Verification Code 2).
This data is required for all CNP (cardholder not present) transactions. In practice all CNP transactions are authorised (and I'm referring to purchase transactions here).
For refunds, there is no requirement for the transaction to be authorised. No authorisation means no CVV needed.

It's my experience with retailers that front line staff are trained by Chinese Whisper, and their process for card transactions are not quite what Visa and MasterCard would wish for.
