Having just gone through this myself, I'll try to throw out what I have found since my last post.
You're only allowed ONE stopover at a North American Gateway. So it has to be on the way out or the way back, but not both. I was under the impression that it has to be the most direct route to a location. Flying to the South Pacific, my North American gateway city could have been SFO, LAX, or HNL (the latter is possible only because there is a direct flight from ORD-HNL). You can, however, have one N. American gateway stopover and one International Gateway stopover. This is how I am able to go from ORD-LAX-NAN (intl' stopover)-SYD-CNS (destination/open jaw)-SYD-LAX-ORD. We were going to have our N.A stopover be in HNL, but decided to extend our time in Australia instead.
You are able to do an open jaw at your destination, although I don't know if it has to stay within the same country or just the same continent. I'm doing an open jaw (arriving in CNS after a stopover at NAN and then departing from SYD), but it's within Australia.
