the MH issue on NWA awards is a different animal. NWA advises the customer of this, and it's even stated at the NWA website:

***WorldPerks members traveling on award tickets on Malaysia Airlines are subject to additional fees that include administrative fees, insurance and fuel surcharges. These fees will be collected by Malaysia Airlines upon check-in.

TG, OTOH, does not do this. What we have here is a rogue agent who took it upon him- or herself to make something of it, and a customer service supervisor who didn't know what they were doing.
