It answers pretty much everything asked about TMO prepaid, including activation kit only, sim + phone, etc. etc. And btw, two of the phones I got were eventually unlocked and can be used worldwide!

(I familiar with most everything TMO prepaid, having used it as my primary carrier for the past 3 years...)
