Well for a really bad, spur of the moment decision, how about turning down an old CW (cradle seat type old) to Concorde IAD-LHR upgrade with overnight accommodation thrown in .... my only excuse was that I was with a bunch of colleagues and we'd just had a good meeting with some US regulator types and were having a bit of a party in the lounge... some decisions are really, really bad
