The only italian airport where you wouldn't have to bus would be FCO and MXP but not as sure about the latter. The walk at FCO is monumental including a shuttle train. LIN is minute, yes there is a bus but once you have cleared passport control 5 minutes and you are back in the departure lounge.
True - the upshot about LIN though is that it is so small that it takes about 1min from arrivals to the bus, and it's a pretty quick ride into the Stazione Centrale. Do this myself regularly.
It is true about fog diversions as noted above...but only in winter/cold autumn days. No problems during summer of course...
