As far as I can remember, I've always had to pay the change fee with new cash, at least on CO. This dates back years. For my tickets issued by the corp travel agent, they are able to deduct it from the value of the tix, but anything I've directly booked, be it rebooked at the same time or a later date, the change fee is new cash.
