I had the same issue yesterday and called in. Itg was strange that it did not show in the itinary but they had taken my miles. Anyways, I called in and they told me it cleared but was in a limbo land status(hehe). They corrected their end and now I have the 1st class seat. Just call in.
No phone call necessary--just patience. I must have been in the same limbo. The upgrade now shows confirmed.
