The fee is paid on arrival at SCL airport. It is "reciprocity fee" for people holding U.S. passports; payback for our high VISA fees to visitors from Chile.
Once you get the "paid" stamp in your passport, you do not have to pay it again for the life of that passport on future visits to Chile.
