Checked bag damaged - what should I do and expect?

Came back from a family vacation last night. When I picked up my checked bagges, I found one of them got dented. Immediately I reported it to a rep. at the laggage service desk. She discussed with her co-worker and offered me to get it repaired. I denied as I would not have the time to leave the bag with them (about 6 weeks). Then, she gave me a half-boarding pass size card titled "Damage Inquiry" with her hand-written note of the date, route flown, and the damage, and told me to mail it to 1k help desk to get compensation. I noticed that they had a full letter size form on the desk but she didn't use it.

This is first time I got my checked in luggage damaged - not severe but do need some repair if I want to continue using it. What shall I do at this point? Just call 1k service number and send this card in?

Any advice would be appreciated.
