Well that, while smarmy, would be the ideal ANYWHERE. As an Aussie I have never found any difficulty (or great expense) in getting comprehensive medical cover for my US trips. There are numerous providers...and while I have been fortunate enough not to need the cover (so far) a friend had a heart attack in the USA and was very happy with the performance of the travel cover purchased here.

Not smarmy at all. I feel confident saying as an American who works in the healthcare system that it is a brutal mess to navigate on a good day. Purchasing insurance is no guarantee of coverage. While another poster is correct that you would not be denied care, the bill that follows may well be eye-popping.

At least with living outside of the US, realistically the worst thats going to happen is getting some nasty letters in the mail. Its basically impossible to actually collect said debts outside of the US (very costly with a high potential of failure) and because health care is private, its not even going to effect said persons admissibility back into the US.

While I dont normally approve of people skipping out on debts, I wont weep any tears for an American hospital.

Unfortunately the providers are not the culprit here. They face enormous pressure by payers (insurers) who are continuously and systematically ratcheting down reimbursement rates, all the while raising premiums on and denying coverage to their own customers (corporations and individuals).

Did you receive treatment at an in-network facility? Was your treatment preauthorized? Is Mercury in retrograde? There are any number of reasons for otherwise legitimate claims to be denied.
