Question about Lifetime Miles

I have some questions regarding the lifetime miles on United:

1) Are these BIS miles or is it including elite bonuses?
2) As I understand it, they must be on UA metal?
3) Can I get lifetime miles even if I don't credit a flight to MP? (I mean only lifetime miles, no redeemable miles)

I think:

1. It excludes elite bonuses
2. Yes, UA metal
3. No only when credited to Mileage Plus
