Help on using miles for an upgrade

Hi. Can anyone please tell me the rules on what classes you have to be ticketed in to use miles to upgrade, both for domestic and international? I know you can't do this from the very lowest classes and agency bulk tickets..

For international remember: HoMe BoY

H/M - More miles
Y/B - Less miles.

All published US fares should qualify.
