I think it's more based on whether they can be bothered, not if they've read the memo. Surely all know about the PDB policy by now, and except for tight turns, legit reasons, etc., the inconsistent offering of PDB's must come down to individual apathy.

I have flown 22 segments so far this year, 17 of which were in first and not a single PDB offered. I am batting zero. Although I would most likely order just water for a PDB, the return to normal service would be nice.

That's about my average as well for domestic flights -- but international has been MUCH better lately -- I even got a nice glass of white wine when I passed on the "champagne" they served as a PDB.

3/7 CLE-MCO No PDB offered. No excuse, the flight finished boarding early.
3/19 MCO-CLE No PDB but I'll give them a break since the inbound flight was 20 minutes late and people were boarding right up until they shut the door.
