The online form for ORC has never worked for me. Just email [email protected], and they will get back to you. Expect 2 weeks between responses and a minimum of 3 weeks to get it sorted out

- Where did you buy the ticket? Direct from United? One of the agency sites (orbitz, travelocity, priceline, etc)? Another airline? Another type of travel agency?
- Did you put your Mileage Plus (MP) number in the reservation?
- Does your MP account number match *exactly* to the name on the ticket? (there are some allowance for variation, but if your MP name says "Bobby Smith" it will not credit if the ticket was "Robert Smith")
- Have you ever had your MP account audited? or how long have you had your MP account? or Have you ever had any other problems getting flight credit?
- Were there any irregular operations around your flight (cancellations, re routing, etc)?
Do I talk directly with United at the airport about it?

The previous answers cover all possibilities. If you still have the return trip on your account/App, it will eliminate the faulty scanning of boarding pass. That is, if you were a no show, the return trip would be automatically cancelled.
Calling MileagePlus service center will resolve this problem. Please add more info if you want to eliminate more possibilities.

I was about to strongly urge the OP to make sure that the return portion of the ticket hasn't been cancelled. It's possible that UA has no record of the OP having taken the outbound flight. OP needs to check both the reservation and the ticket to make sure that everything is OK.

Wierd Situation
My wife and I took a Star-Alliance Partner flight one-way, on an itinerary that did not include UA. Waited weeks, and no credit shown for either me or my wife. So tried filling out the UA web form, and got the message that our E-ticket numbers were not correct. Contacted the Partner, and they emailed UA.
Eventually, UA emails me and says my wife's account is now credited, but fails to mention my account. So I email them back and say "What about my account?" Have not heard anything.
Any thoughts? TIA

Requesting missing miles form does not seem to be working
Requesting missing miles for Preferred Partner Avianca UIO-SCY-UIO. These miles and PQP correctly posted to my husband's UA MP account.
I entered my Avianca ticket number in United's web form (fyi: I learned that the the last two digits of my eticket were not needed and should not be included--only the first 14).
After entering the ticket number and clicking submit, the form populated with information about my flight. Except, the information was not quite correct and could not be altered. Example: Flight number was shown as 0168 when it was actually 1682. Class was shown as Y. However when my husband's miles correctly posted to his mileageplus account, the class was shown as E.
Only the first segment appeared on the "Request Missing Miles" form, but I think that is normal and I was allowed to add the next segment (SCY-UIO). I did so and then clicked "Request Credit." It took me to a page that had a heading of items requested, but no list!! See attachment. I tried this in both Chrome and Edge with the same result. I also did not receive an email acknowledging my request.
When I previously requested credit, I used the "email process" because I did not understand why my ticket number was not working in the form. UA did not seem to read my email request or understand when I said that my husband had received credit and answered with a canned response saying my flight was not eligible. I did check the miles on the awesome PQP/PQF + RDM Calculator and they are eligible and equal what my husband received. We confirmed our mileageplus numbers were in the computer at both UIO and SCY and even received an upgrade to business on the second segment (assuming due to our *A status--two of our friends were also upgraded). I sent another email with similar results.
I called Avianca and they confirmed my reservation had my UA number attached and even read it to me. (That was also where I learned that the actual ticket number was the first 14 numbers). Thanks for reading this far...I wanted to report the problems I was having with the form and ask if anyone else is having trouble?
I guess my next step will be to call, as the email process if ridiculously frustrating and seems geared to not helping people, but am also wondering if anyone has any other suggestions. It's a small amount of miles, but am having a hard time maintaining status and every PQP counts...

update on my Avianca flight posting to UA MP
I called UA and after 55 minutes they were able to post the miles, PQP and flights to my account. What happened: They were able to see that I had indeed flown on Avianca but the system was not allowing anything to post because of the class of the flight--even though UA's own chart said that I should receive credit. (No explanation as to why my husband's miles posted and mine did not.) They told me to send a copy of my receipt (would have been a total pain in that I would have had to work with the Travel Agent to break the flight out of our overall invoice.) I kept (nicely) saying that if their software wasn't allowing a legitimate flight to post, that this would be a problem for many of their customers and not just me. Pearl, who was helping me, went to ask the supervisor again about how to make it work and was finally able to get it done.

None of the flights from a recent Air Canada itinerary posted for me. I fly AC a lot and normally it posts exactly 3 business days after the flight.
After attempting the automated form on United's website, it produced two distinct errors:
1. After entering the ticket number, it claimed the ticket was not used or had been exchanged.
2. After entering the full details of each flight, it claimed the flights weren't eligible for credit due to fare class but the fare class displayed wasn't the fare class I entered nor one that even exists on Air Canada. The actual fare class on ticket receipt is eligible.
I tried contacting a chat agent who simply told me they couldn't do anything about non UA flights so I sent in a written request with all supporting documentation and am waiting to hear back.
Bizarre issue as I've had hundreds of AC flights credit correctly.
