Many of these are EAS (Essential Air Service) markets subsidized by the Federal government.
Since service is dependent on the subsidy, there is no competition.
One of these markets I have fly into is PQI in northern Maine, currently served by PenAir of Alaska (although the fleet and crews are unchanged since this service was provided by US Airways Express).

A handful of years ago EAU used to have service from nearby CWA, on a UA code operated by UX probably. It was the shortest domestic route I had seen on the gate departure board that year, although I'm not sure who would fly that and what the TSA arrangements were for passenger screening for that flight.

STC = St Cloud, MN. It used to have NW, and perhaps DL after GE merger for a bit, commuter service to MSP, but that was dropped a while ago. More recently UA has given them scheduled service again, to/from ORD of course.
