My list isn't that long:
US
Canada
England
West Germany
East Germany
Poland
Soviet Union
Denmark
Norway
Sweden
Finland
Australia
Let me add that all of these (except US and the East Block countries) are with a manual transmission. Stick shift in most places, but most notably Australia with a steering column shift. Imagine that on the "wrong" side of the steering wheel!

For everywhere else... Rail is always my preferred mode of transport for long distances, two wheels and pedals for the shorter ones.

I can only speak for myself, but I simply wouldn't think of car hire/rental as a mode of transport when abroad, unless it were absolutely essential, such as in much of the US, or when I'm visiting particularly rural areas (even then, it's country dependent). Otherwise, I only ever use a car or van at home when I need to transport something.

While your modus operandi might work well in most of Europe you would be doing a lot of pedalling in Africa or the Americas.
