What's the cheapest way to send money/cash?

Western Union charges a staggered rate between 0-1000 with $1000 costing $80 in fees. Are there other options?

Try Pay Pal

If you both have or set up a paypal account, you could transfer $ very cheaply. It might even be free, but you'd have to check that out.

domestically and internationally; Paypal is too slow. I'm asking about services that provide instantaneous access like Western Union. That's the only one I can come up with now.

If it is on a regular basis, and the recipient has access to an ATM, open a bank account (using the same bank as the ATM that the recipient has access to if possible, or at a bank with the lowest other-bank ATM use fee otherwise) and send the recipient an ATM card (call the recipient to give the password). Then you just deposit the money in the account, and the recipient takes it out of the ATM.

Is there any fees associated with Paypal Debit i.e. withdrawal fees/atm fees?

You can also use pre-paid mastercard or AMEX check cards.

they are easy to buy and easy to refill.

If you have the same bank on both ends, you can do account to
account transfer.

You're right that Western Union is probably the most common. Banks can arrange wire of Swift transfers for a small fee. In the US, there are reporting requirements if you transfer more than $10,000.

Online Poker site

IE.. Pokerstars uses neteller and pays the fees. It would be a longer process but if you use pokerstars to transfer money into a poker account then use their transfer Real money option to another player which is the one you want to get the money then have them withdraw the cash. Its a little longer than western unioon but its completly free!! I love workarounds!
