I received the $750 in vouchers that I had asked for (I asked for half the $1500 out-of-pocket cost). I plan to use them this summer to take the kids somewhere.
I learned another lesson -- never pay a bill in full without making them file a claim for you first. I paid the $500 ambulance bill and filled in the insurance information. They deposited my check and did nothing with the insurance. Now I have to file a claim myself with the insurance company (crummy benefits but better than nothing).
