Part of the problem with information not being in the PNR/record can be caused if the issuing travel agent/airline does not share the same GDS system as the airline you are flying on. More and more airlines are asking for ticket numbers to be put into the record. If they do not see them, your reservation gets canceled.

Since most airlines use a different system for ticketing they sometimes cannot see the ticket number in the record or any of the other information they would need from the issuing party. This is why there is still a paper ticket. Otherwise some travel agencies would not be able to issue tickets on some carriers while other agencies could. It would get even more confusing that way.
