How design fails increase risk of pilot error
 

The San Francisco Chronicle has an interesting article about how the design of local airport SFO reduces safety, apparently there are both runways 01 and 10, and increases the risk of pilots incorrectly programming their planes. Apparently this is a rare, but not unheard of, problem in the industry. We know that this happens to passengers, too, including the infamous 1985 story of a man who flew to Auckland, NZ instead of Oakland, CA.

Just think it is interesting to consider how design affects safety, and service. I, for one, would love to see how design thinking could reduce pain and increase safety + security for travelers. Does the FlyerTalk community have any ideas for how to improve design for pilots, crew and passengers?
