I once had a friend ask me to book a ticket for him online for a flight we were on together. Somehow, he ended up listed as "Master," :) which BTW is still listed as one of the few title options by both AA and UA.

the prefix field could serve a purpose to identify if there's a doctor on the flight if needed

Perhaps, but a Doctor of Laws or Philosophy won't be of much use when someone is having a heart attack at 37,000 ft!

I think so. I seem to remember paying for that when I was ordained and obtained a Doctor of Divinity degree on the inter tubes some years ago.

Yes. My assistants have over the years put doctor in my profiles from various places like car services. I don’t pay much attention to it. But I was very careful to go out of my way to make sure they didn’t do it on airlines. Someone having a medical emergency in flight is very unlikely to have a resolvable genetics issue.
