Another tipping question

We are flying into Prague, CZ going on a riverboat cruise, and departing from Budapest. We are arranging for prepaid transfers (private) for airport - hotel in both locations. Is it customary to tip the drivers, and how much? The transfer in Prague costs us $27, and we might not have Kronas yet (will get Kronas at the hotel ATM).

Also, is it customary to leave cash money for the housekeeping staff in Prague and Budapest, or are they paid a living wage?
