My pet peeves are: Big bottom towels -- I wish hotels would have towels for big bottoms!!!
But my absolute biggest peeve is having to pay for internet and/or parking when I am a rewards club member at large well-known hotel chains! I have stayed at some very nice smaller chains and get these amenities for free!

Poor heating and air conditioning systems.
Most are ridiculously noisy, drafty, and there is no clear way to set the desired temperature. The through- the-wall units are very poor; outside of the U.S., they use split mini systems which are far superior.
Useless Internet connections.
Either they won't work at all or are ridiculously slow. Most are insecure; they have no password protection.
Noise.
It should not be possible to hear plumbing noises from other units.

Anything that keeps me from sleeping, obviously, is annoying. The hotel can't help other guests being loud (other than having thicker walls), but when the curtains won't actually block out light, that's terrible. Everyone else has listed the wide variety of problems here.
But the thing that irks me way more than it should is hotel waste, especially when they've got 40 cards spread across the room talking about all the good environmental savings they want you to help them with. Then they wash towels you hang up anyways, throw away a once-used bar of soap (every day, on a 10 day stay, until I start hiding it), and the maids leave the TV on playing smooth Jazz all day after they clean the room.

My main complaint is expensive hotels charging for WiFi/Internet access.
They should provide it for free, build it into their prices.
If the cheaper hotel chains can do it (most of them do provide free Internet access), why can't the more expesnive chains?

Just bring along old key cards from other hotels to use in the slot. Even business cards or other thick card stock can work.

Getting your key card, walking all the way up to your room with all your luggage, key card doesn't work.
Somebody elses stuff in your new room.
The guy next doors TV. I don't watch TV, I don't want to hear anyone elses.
Cooling/heating systems that can't be turned off.
The 12:00 noon on a weekend call from house keeping asking if they should ignore your "Do not disturb" sign. I slept in for a reason, usually because I have had a long damned week and am in a timezone 8 or 9 hours from the one I live in. Leave me alone.
Rooms next to stairwells.

Glasses brought up from the bar (for ice) that are never collected by housekeeping.

Towels that are too small. I'm not a big person by any means but I do like a towel that I can wrap around my body. Mr LadyB is bigger than me, he has no chance of a towel going all the way round his body. I also like to have a hand towel in a bathroom, 2 bath towels doesn't cut it.......
Noise from other inconsiderate people - recently the people staying in the room next to my daughter had their music on so loud, she couldn't hear her tv, it turned out they had gone out & left their music on very loud - the hotel staff dealt with it very quickly so a ^ to them.
Seeing dirty crockery left in corridors - why is it there? It looks dirty and like no-one could be bothered to move it.
One cup for two people, or two miserly tea bags on the hospitatlity tray, we are likely to want more than one cup of tea without having to request more tea bags from reception.
Pillows are a bug bear as well - I can't bear flat pillows that feel as if it isn't even there, like you're sleeping on the mattress....

When you book a room, the price they quote is the price you should pay. If every room has a "resort fee" (even if you never use the beach, pool, gym, etc.), then that is part of the room rate.
Every traveler today uses WiFi. It is free at the motels along the road, and in the coffee shop next door to the hotel. If you are in range, and are a Comcast subscriber, you can be connected on the street. Include internet in the cost of the room! Charges of eight to ten dollars a day (or more) are outrageous and unjustified. Why is it that the more expensive the hotel, the more you are charged for the same internet service?
Why not charge for the pillows, or the carpet on the room floor if you are going to charge for essentials? Hotels that add these charges are getting more and more like RyanAir.
Let the hotel figure out what they want to sell a room for, and make that the price.
2: "Resort Fee"
When you book a room, the price they quote is the price you should pay. If every room has a "resort fee" (even if you never use the beach, pool, gym, etc.), then that is part of the room rate.

Besides so many things already covered above...
Alarm clocks with preset station buttons that are often not set to functioning frequencies, and which don't let you set your alarm to anything but a preset station. Same clocks are often off by at least a few minutes and have no discernible way for guests to set them. I'm looking at you, Hampton Inn.

Location: LAS - where you can get married and divorced in the same 24 hour period. Perfect for the woman who's saving herself for marriage and the man who wants a one night stand.

I hate fees - like an energy charge of $3.00/day. It probably goes to pay for the GM's mistress.
I have a remote that is programmable so I can get to the setup menu on a TV. I change the PPV channels to be part of the regular listing and usually there is a 24 hr porn channel. Then I change the barker channel (the channel that comes on when the TV is turned on) to the porn channel. When the TV is turned on it is the porn channel (now free). Most guys won't mind but at some point some woman is going to check in with their 6 year old who asks what the woman on TV is doing and that woman will give the GM an earfull. I don;t have the time or energy to complain about the $3.00.day fee but sometime someone else can get the GM wound up.

Because they can get away with it. The more expensive the hotel, the more likely it is that the guest is not the one actually paying for the room + additional charges, or is less sensitive to "small" additional charges. Still bugs, me, though

I hate maids (Guest Room Attendants) that ignore the DND sign - they think it means knock louder. I used to travel with entertainers so we would stay up until 4 or 5 am (1 or 2 Pacific time). Usually we were in a room for a week to 10 days. The maid would start knocking at about 10 am - every day. I found if I answered the door naked and asked - "are you the broad the bellman seent up? Come on in we're all waiting." They would run away, and only leave towles outside which is all I realy wanted in the first place.
