Travel insurance companies and zero coverage insurance?

Travel insurance companies and zero coverage insurance?

1) What are some good travel ins companies? Allianz seems integrated with the travel booking - any good?
You could also purchase the Travel insurance for a period as opposed to a specific trip - guess this is better if you have multiple trips?

2) Who provides the so called "zero coverage insurance" - sorry if my terminology is wrong!
The idea seems that whatever the CC does not cover will be covered by this ins. And it cost minimal amount. Pls correct me if I am wrong?
Again who provides it?
